Located within the State-of-the-Art Westmead Health Precinct within the Renal Supportive Care team, The Social Worker is an important part of this dedicated multidisciplinary team supporting patients and families across the renal care journey.

What you'll be doing
As a Senior Social Worker, you'll play a pivotal role in supporting clients and families through their renal journey.
You'll provide specialist social work assessment, counselling, advocacy, education and practical support to renal patients and their families. Using advanced clinical reasoning and a psychosocial approach, you'll help people navigate difficult decisions and changing circumstances with dignity, choice and support.
In this role you'll:
Provide autonomous, high-level social work intervention across all stages of Renal Supportive Care, from treatment decision-making through to end-of-life and bereavement support.
Support patients and families with advance care planning, guardianship, wills, funeral planning and access to relevant services and information.
Participate in multidisciplinary case conferences, clinical reviews, ward meetings and education programs to promote integrated care.
Advocate for patients, coordinate services and work collaboratively with health professionals to respond to complex psychosocial needs.
Contribute to education, supervision, research, evidence-based practice and quality improvement while supporting students and strengthening social work practice.

License: Social Worker.
Qualification: Degree in Social Work with eligibility for membership of the Australian Association of Social Work (AASW).
Selection Criteria:
Demonstrated high levels of clinical experience possessing extensive specialist knowledge in Renal and/ or Palliative Care Social work including knowledge and experience in advanced care planning, the psychosocial aspects of caring for dying patients and their families, grief counselling and community support for those impacted by chronic illness and increasing decline in health status.
Demonstrated experience in staff and/or student supervision.
Experience in developing and implementing policies and procedures for service improvement and skills in providing training and education.
Well-developed time management skills with excellent written and verbal communication, including report writing skills and high-level computer literacy.
Demonstrated excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders, throughout all organisational levels.
Demonstrated ability to initiate, lead, complete and implement quality improvement initiatives and service evaluation processes and lead ongoing quality improvement activities with other staff.
Demonstrated understanding of issues associated with working with Culturally and Linguistically Diverse client group.
