Location:Cumberland Hospital (May be required to work across WSLHD Drug Health)

Where you'll be working
Join the dedicated Drug Health Services team at Cumberland Hospital, where you'll play an essential role in supporting individuals receiving opioid treatment. Working alongside experienced multidisciplinary professionals, you'll provide compassionate, evidence-based care that empowers patients to achieve better health outcomes in a supportive and inclusive environment. You may also be required to work across Western Sydney Local Health District Drug Health Services to support service delivery and meet the evolving needs of the community.

What you'll be doing
As a Registered Nurse Case Manager, you will provide safe, high-quality nursing care while coordinating opioid treatment services for individuals, carers and families. You'll work collaboratively with the multidisciplinary team to develop, implement and evaluate care plans that promote recovery, wellbeing and best practice.

  • Manage a caseload and coordinate evidence-based opioid treatment services.

  • Develop, implement and evaluate individualised patient-centred care plans.

  • Collaborate with multidisciplinary teams to achieve optimal patient outcomes.

  • Advocate for diverse communities by delivering culturally safe and inclusive care.

  • Support colleagues through education while maintaining professional and legislative standards.

Essential Criteria

  • Current Nursing Registration with the Australian Health Practitioner Regulation Agency (AHPRA)

Selection Criteria

  1. Skills and experience in patient case management

  2. Able to demonstrate effective communication and interpersonal skills

  3. Demonstrated ability to liaise effectively within a diverse multidisciplinary team

  4. Non judgmental attitude with patients

  5. Knowledge of drug and alcohol dependence issues and current treatment interventions

  6. Current NSW Drivers Licence
