Western Sydney Local Health District's Hospital in the Home (HITH) service provides high-quality acute care to patients in the comfort of their own homes, reducing the need for prolonged hospital stays while maintaining excellent clinical outcomes. Based at Cumberland Hospital, you will join a collaborative and highly skilled multidisciplinary team committed to delivering safe, evidence-based and patient-centred care across Western Sydney.

What you'll be doing
As a Registered Nurse within the Hospital in the Home service, you will provide comprehensive nursing care that is evidence-informed, person-centred and focused on achieving positive health outcomes. Working collaboratively with patients, carers and multidisciplinary teams, you will assess, plan, implement and evaluate care in a dynamic community-based environment.
Key responsibilities include:
Delivering high-quality nursing care through comprehensive assessment, care planning and clinical evaluation
Coordinating and documenting patient care in accordance with professional and organisational standards
Collaborating with patients, carers and multidisciplinary teams to achieve optimal care outcomes
Supporting quality improvement initiatives and promoting evidence-based nursing practice
Contributing to the education, development and support of nursing colleagues and students

Essential Criteria:
Registration: Currently authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (APHRA)
Selection Criteria:
1. Current unrestricted Drivers License
2. Recent relevant experience in clinical setting as deemed appropriate by WSLHD
3. Excellent time management and organisation skills with attention to detail
4. Evidence of computer literacy and understanding of relevant health related information systems
5. Effective interpersonal skills with a demonstrated ability to develop and maintain relationships with clients, carers and the wider health care team
