About the Role We are seeking a compassionate, reliable, and person-centred Support Worker to provide dedicated support to a participant living in Wangratta, Victoria This role focuses on supporting an individual and their family to live a meaningful, independent, and fulfilling life while promoting choice, dignity, inclusion, and human rights. You will build a strong professional relationship with the participant, their family members, and friends, whils also supporting their daily living activities, community participation, health and well-being, and personal goals. The successful candidate will demonstrate empathy, patience, flexibility, and a genuine commitment to empowering people with disability to achieve the life they want and choose. Delivering high-quality support services to customers with disabilities. Assisting clients in their daily living activities, such as personal care, mobility, and skill development. Promoting community integration and supporting clients in accessing social and recreational activities. Collaborating with a multidisciplinary team to develop and implement individualised support plans. Encouraging independence and empowering clients to achieve their personal goals. Advocating for the rights and needs of individuals with disabilities. What You'll Do Person-Centred Support Provide individualized support tailored to the participant's needs, preferences, goals, and aspirations. Promote independence, self-determination, and informed decision-making. Support the participant to engage in activities of interest and maintain meaningful relationships. Encourage active participation in daily routines and community life. Respect the participant's rights, privacy, dignity, cultural background, and personal choices. Personal Care and Daily Living Support Assist with personal care tasks as required, including showering, grooming, dressing, toileting, and hygiene support. Support meal preparation and healthy eating practices. Assist with household tasks such as cleaning, laundry, and maintaining a safe home environment. Encourage and develop independent living skills wherever possible. Safety and Compliance Follow all organizational policies, NDIS Practice Standards, and relevant legislation. Maintain a safe working environment for yourself and the participant. Report any concerns relating to abuse, neglect, exploitation, or risks to well-being.
