The Mootang Tarimi Outreach Program supports Aboriginal and Torres Strait Islander people through culturally safe, patient centred care and engagement across the healthcare journey. Working collaboratively with multidisciplinary teams, community services, patients, families and carers, you will play a key role in improving health outcomes and reducing barriers to accessing healthcare.

What you will bring to the role
- Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated effective communication through the use of information technology and sound written, verbal and interpersonal skills
- Demonstrated knowledge of chronic disease prevention and self-management principles.
- Aboriginal cultural awareness and sensitivity with knowledge of the issues affecting the health of Aboriginal people.
- Demonstrated ability and experience in building and maintaining positive partnerships and networks for delivering health services
- Demonstrated understanding and commitment to quality improvement principles.
