The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth).
Are you passionate about improving health outcomes for Aboriginal children and young people and making a meaningful difference in your community? We are seeking an Aboriginal Health Worker to join the Out-of-Home Care Health Pathway Program (OOHC HPP) multidisciplinary team.
In this rewarding role, you will provide culturally responsive assessment, engagement and care coordination for children and young people in Out-of-Home Care. Working closely with health professionals, carers, families and partner agencies, you will help ensure cultural identity, safety and individual needs are recognised, respected and embedded throughout health assessment, care planning and service delivery.
The Aboriginal Health Worker plays a vital role in reducing health inequities experienced by children and young people in care, particularly Aboriginal children and young people who may have experienced trauma, abuse, violence or neglect. Through advocacy, relationship building and culturally safe practice, you will support improved access to health services and help navigate a coordinated pathway through the healthcare system, contributing to better health and wellbeing outcomes.

What you will bring to the role
- This is an Aboriginal Identified position. Nepean Blue Mountains Local Health District considers that being of Aboriginal descent is a genuine occupational qualification under section 14 of the Anti-discrimination Act 1977 (NSW)
- Possession of a Certificate III in Aboriginal Primary Health Care or a demonstrated willingness and capacity to undertake this qualification within the first 12 months of employment, together with relevant experience working in Aboriginal Health and community services.
- Demonstrated high-level knowledge and understanding of Aboriginal culture, history and contemporary issues, and the ability to apply culturally safe practice principles within a health and multidisciplinary team environment.
- Demonstrated ability to establish and maintain strong, respectful connections with Aboriginal communities, Aboriginal Community Controlled Health Organisation and mainstream organisations to support access to health and related services.
- Demonstrated experience working with Aboriginal children and young people who have complex health and social needs, particularly those involved in or affected by vulnerability, trauma or statutory out-of-home care systems.
- Strong interpersonal and communication skills, with the demonstrated ability to build trusting, respectful relationships with Aboriginal children, young people, carers, families and external agencies, and a current driver's licence with willingness to travel across the LHD for work purposes, which may include long distances.
- Demonstrated ability to provide advice, support and advocacy within a multidisciplinary team, including advocating for the cultural, health and social needs of Aboriginal children and young people within a complex health system.
- Sound knowledge and awareness of NSW child protection legislation, policy and practice frameworks and the ability to support their application in a culturally safe and responsive manner when working with Aboriginal children and families.
Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa.
