What you'll be doing
As an Aboriginal Health Practitioner - Chronic and Complex Care, you will provide flexible, holistic and culturally appropriate healthcare services to Aboriginal people living with chronic and complex health conditions. Working within your Scope of Practice and alongside registered health professionals, you will support individuals, families and communities to achieve better health outcomes and improve access to healthcare services.
Conduct assessments, health checks and clinical procedures to support care planning and management of chronic and complex conditions
Deliver health promotion, education and disease prevention initiatives that empower people to manage their health and wellbeing
Coordinate and contribute to culturally appropriate care plans in collaboration with clients, families, carers and multidisciplinary teams
Engage with local Aboriginal communities to develop, implement and evaluate health programs and strategies
Maintain accurate clinical documentation and health records in accordance with legislative and organisational requirements

Essential Criteria:
Qualification: Certificate IV in Aboriginal Health Care (Practice)
Registration: Current Registration with AHPRA as an Aboriginal health practitioner
Evidence of Diversity claim: Aboriginal /Torres Strait Islander
Selection Criteria:
1. Aboriginal and/or Torres Strait Islander descent. (N.B. An applicant's race is a genuine occupational Qualification and is authorised under Section 14 of the Anti-Discrimination Act 1977, NSW).
2. Certificate IV Aboriginal Primary Health Care (Practice) with current registration as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency.
3. Demonstrated experience in working with multidisciplinary team and to liaise with other health professionals and external agencies to facilitate integrated care for Aboriginal people.
4. Demonstrated ability to undertake screening assessments, pathology and other forms as clinical assessments relating to Aboriginal Chronic and complex care
5. Demonstrated oral and written communication skills, time management, and computer literacy including word processing and data entry skills.
6. Active involvement in the local Aboriginal community, with demonstrated understanding and commitment to improving the health issues impacting on Aboriginal communities.
7. Ability to work unsupervised, use own initiative and take a lead role in initiating and carrying out community screening activities.
8. Current valid Driver Licence
