An exciting opportunity exists for an experienced and motivated Clinical Nurse Consultant Grade 2 to lead an innovative Heart Failure Supportive Care project across Northern Sydney Local Health District. In this nurse-led role, you will work collaboratively with a multidisciplinary team to enhance the delivery of holistic, patient-centred care for people living with advanced heart failure. The successful candidate will provide clinical leadership, care coordination, service development, and support for patients and their families, helping to improve quality of life and access to supportive care services across the district.

Candidates will need to meet the following criteria:
- A Registered Nurse who has a minimum of 5 years full time post graduate experience, with at least 3 years full-time equivalent experience working in Cardiology within an acute hospital environment. Additionally, the applicant must have relevant post registration qualifications (minimum Grad Diploma/Certificate level) or evidence of working towards a Master's level qualification in Cardiology or other relevant speciality.
- Demonstrated clinical leadership skills and an ability to work collaboratively with patients, carers and the multidisciplinary team with a demonstrated commitment to excellence in nursing practice.
- Demonstrated effective communication and interpersonal skills across a variety of mediums and an understanding of the practice development framework.
- Demonstrated high level clinical experience and leadership, competency and expertise in heart failure including experience in the assessment and management of patients and their families with complex needs.
- Effective organisational, time management, research and computer skills and proven ability to assess, plan, initiate and manage complex cases and work towards goals and targets.
- Excellent interpersonal skills and communication, including a demonstrated ability to maintain and establish networks across multiple disciplines within the LHD.
- Demonstrated commitment to personal and professional development and quality improvement.
- Current driver's licence.
