Noble Park location on Boonwurrung Country Part time (0.7FTE), Permanent role Workdays can be flexible - Tuesday is a preferred workday. $105K + Super and Salary packaging (pro rata) The Opportunity Join our Adolescent Support Program and make a lasting difference in the lives of vulnerable young people aged 12-17 and their families. Based in Noble Park, this outreach-focused role works with young people who may be at risk of entering out-of-home care, helping them build stability, strengthen family connections and access the support they need to thrive. As a Case Manager, you will provide intensive case management, assessment, advocacy and support to a caseload of young people. Working collaboratively with families, schools, Child Protection and community services, you will play a critical role in helping young people achieve positive outcomes while remaining connected to their family and community wherever possible. This is an opportunity to join a dedicated team committed to strengths-based, culturally safe and person-centred practice. Key Responsibilities Case work of young people (including some clients on statutory protective orders, in accordance with Children's Court directions and voluntary clients referred from both Child Protection and the broader community) Assist young people and their families to minimise risk and gain stability Promote the maintenance of the young person with their family, extended family and other support networks Develop stakeholder relationships within the community and be an effective advocate. Promote culturally responsive and inclusive practices for Aboriginal and Torres Strait Islander young people, families and culturally diverse communities. About you To be successful in this role, you will bring a passion for working with young people and families, alongside strong case management and relationship-building skills. You are adaptable, resilient and thrive in a dynamic outreach environment. A Bachelor qualification (or higher) in Youth Work, Social Work, Community Welfare or a related discipline. Experience working with young people and an understanding of adolescent development and the challenges impacting vulnerable young people and families. Knowledge of strengths-based and youth-focused intervention approaches. Experience undertaking assessments, case management and coordinated support planning. Strong organisational skills and the ability to manage competing priorities in a fast-paced environment. The ability to build and maintain productive relationships with young people, families, colleagues and external stakeholders.
