About the Role As Primary Care Practice Nurse, you will provide safe, effective and person-centred nursing care to community members accessing two existing GP Clinic's working collaboratively with GP's and the broader multidisciplinary team, you will play a key role in supporting patient care, health promotion and service delivery. Key responsibilities include: Clinical assessment and patient observations Wound care and procedural assistance Immunisation delivery Supporting GP telehealth consultations Escalating patient care when required Care planning and coordination Health promotion and community education Maintaining clinical equipment and supplies Supporting quality improvement initiatives Results follow up Supporting patients to engage with a long-term Primary care provider About You Ability to role model the values of KDHS, Empathy, Wellbeing and Community Demonstrated advanced clinical nursing skills and knowledge in acute and aged care context, with a commitment to the principles of quality management and evidence-based practice in nursing Possess highly developed interpersonal, and written and verbal communication skills, including conflict resolution strategies. Demonstrated ability to work collaboratively as part of a multi-disciplinary team Ability to manage complex processes and multiple projects, demonstrating advanced work organisation and co-ordination Demonstrated computer literacy skills Current practice in a clinical setting providing direct patient care. Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse Minimum 5 years' post-graduate nursing experience You will bring: Current AHPRA registration as a Registered Nurse Div 1 Experience in an Urgent Care Centre, general practice, or primary care settings Strong communication and organisational skills Confidence using the electronic clinical system Best Practice and digital health platforms Nurse Immunisation certification or a willingness to complete same Desirable experience includes: Experience in working with Telehealth care delivery models Experience in the use of Best Practice and Hot Doc software Understanding of Medicare Benefits Schedule (MBS) item numbers and billing processes Knowledge of RACGP Standards What KDHS Can Offer You KDHS is a compassionate, family friendly employer where every employee is a valued team member.
