An opportunity exists for a motivated Nurse Practitioner to join the Virtual Aged Care Service at Nepean Hospital. This innovative role offers the chance to lead and deliver high-quality, person-centred care to older people through a contemporary virtual model, helping residents of Residential Aged Care Facilities (RACFs) receive timely clinical support and avoid unnecessary hospital presentations.

As a Nurse Practitioner within the Virtual Aged Care Service, you will work collaboratively with Multidisciplinary teams, Aged Care providers, General Practitioners, Specialists and Hospital Clinicians to provide expert assessment, diagnosis, treatment, care planning and care coordination for older people with complex health needs.
This is a role that will see you provide advanced and autonomous clinical care through virtual and innovative models of service delivery, including telehealth consultations, clinical decision-making, prescribing within scope of practice and care coordination.

The successful applicant will play a key leadership role in supporting Aged Care facilities and Health Professionals by delivering specialist expertise, promoting best practice and improving health outcomes for older people across the district.

What you will bring to the role

  1. Proven commitment to ongoing development of safe clinical practice to an advanced level with a successful track record of initiating and leading ongoing education initiatives to improve evidence-based clinical practice and practice development.
  2. High level of verbal and written communication skills with the ability to work autonomously or as part of a team.
  3. Demonstrate evidence of advanced practise and clinical leadership within the aged care setting.
  4. Current Registered Nurse/Midwife appointed as such and who is endorsed by the Nurses and Midwives Board of NSW (AHPRA) to practise as a Nurse/Midwife Practitioner.
  5. Current Drivers Licence valid in Australia
  6. Adhere to the NBMLHD and NSW core values.
  7. Demonstrate expert knowledge and application of skills in contemporary Nursing practise.
  8. Work collaboratively with others and bring together patients, consumers, carers and the wider health team to achieve goals
