Apply Job no: CGFJR Category: Clinical, Clinical Care Coordinator You will work closely with the Home Manager, coordinating clinical operations, supporting staff development, monitoring compliance, and promoting a culture of continuous improvement. About the Role As the Clinical Care Coordinator at Parkdale Aged Care, you will provide clinical leadership and oversight across the home, ensuring residents receive safe, high-quality, evidence-based care in line with the Aged Care Quality Standards. Key Responsibilities Lead and coordinate the delivery of high-quality clinical care. Complete and oversee comprehensive resident assessments and care planning. Provide clinical leadership, mentoring, and support to Registered Nurses, Enrolled Nurses, and Personal Care Workers. Coordinate specialised clinical care, including wound management, dementia care, diabetes management, and palliative care. Oversee infection prevention and control practices and ensure compliance with clinical standards. Monitor resident outcomes and implement evidence-based improvements to care. Ensure clinical documentation is accurate, timely, and compliant. Identify and manage clinical risks while promoting resident safety and wellbeing. Liaise with GPs, allied health professionals, residents, and families to coordinate holistic care. Participate in quality improvement initiatives, audits, accreditation activities, and continuous improvement projects. Support education, coaching, and professional development of the clinical team. About You You are an experienced Registered Nurse with strong clinical knowledge, excellent leadership skills, and a genuine passion for aged care. You will bring: Current AHPRA registration as a Registered Nurse. Minimum three years' experience in residential aged care. Demonstrated leadership and mentoring experience. Strong understanding of the Aged Care Quality Standards and clinical governance. Excellent clinical assessment and decision-making skills. Outstanding communication and relationship-building abilities. Strong organisational and time management skills. Commitment to resident-centred care and continuous improvement. Highly Desirable Infection Prevention and Control (IPC) Lead qualification. Postgraduate qualifications in Gerontology, Dementia Care, Palliative Care, or a related field.
