About the Role

We are seeking a compassionate and motivated Registered Nurse to join our Community Health Nursing team. This is an excellent opportunity to work within a skilled and diverse group of healthcare professionals who are committed to delivering high-quality, person-centred care across the community.

Based from Community Health Centres in Newcastle and Raymond Terrace, you will provide nursing care to clients in their homes and clinic settings throughout the Hunter and Lower Mid North Coast regions. You will play a vital role in supporting individuals to maintain their health, independence, and wellbeing while receiving care close to home.

  • Deliver safe, evidence-based, person-centred nursing care to clients in community and clinic settings
  • Establish and maintain therapeutic and professional relationships with clients, families, and carers
  • Conduct comprehensive nursing assessments and develop individualized care plans
  • Implement, monitor, and evaluate nursing interventions to achieve positive health outcomes
  • Collaborate with multidisciplinary healthcare professionals to provide coordinated and holistic care
  • Promote health education, prevention, and self-management strategies
  • Maintain accurate and timely clinical documentation using electronic health record systems
  • Support clients to remain healthy, independent, and well within their homes and communities


About You

To be successful in this role, you will demonstrate:

  • Current registration as a Registered Nurse with the relevant nursing regulatory body
  • A genuine commitment to providing high-quality, client-focused care
  • Strong communication, interpersonal, and relationship-building skills
  • Effective time management and organisational abilities
  • The ability to work independently and make sound clinical decisions in a community setting
  • Confidence using computer systems and electronic documentation platforms
  • A willingness to learn, accept support, and develop community nursing expertise
  • A collaborative approach and commitment to working within a multidisciplinary team

Desirable Skills and Experience

  • Previous experience in community health, primary care, aged care, or chronic disease management
  • Knowledge or experience in wound care, catheter management, and health promotion
  • Understanding of person-centred care principles and contemporary nursing practice
