Make a Meaningful Difference in Community Mental Health

We are seeking motivated, recovery-oriented Registered Nurses to join our Community Mental Health team in Coffs Harbour.

This is an exciting opportunity to become part of the Adult Mental Health Service as we transition to a contemporary Model of Care built around clearly defined care pathways, person-centred practice, and enhanced clinician autonomy.

About the Role

As a Registered Nurse in Community Mental Health, you will provide high-quality, evidence-based care to adults experiencing acute and chronic mental health conditions, including individuals with complex psychosocial needs.

Working as a key worker within a multidisciplinary team, you will coordinate care and support consumers through the following care pathways:

  • Brief Interventions
  • Specialist Assessment
  • Care Coordination
  • Rehabilitation and Recovery

You will deliver timely, responsive, and recovery-oriented care that supports consumers to achieve their goals while promoting safety, wellbeing, and independence.

Key Responsibilities

  • Coordinate and deliver care as a key worker across the four care pathways.
  • Participate in intake meetings and contribute to pathway allocation decisions.
  • Undertake mental health triage and comprehensive clinical assessments.
  • Develop collaborative care plans, safety plans, and relapse prevention strategies.
  • Provide therapeutic interventions within community settings.
  • Conduct physical health and metabolic monitoring in line with best-practice guidelines.
  • Facilitate smooth transitions between inpatient and community services, including post-discharge follow-up.
  • Liaise with consumers, carers, families, GPs, NGOs, and other service providers to ensure coordinated care.
  • Participate in multidisciplinary clinical reviews and case discussions.
  • Administer long-acting depot injections through the Depot Clinic
  • Provide relief support to the Clozapine (Clopine) Clinic as required.
  • Contribute to continuous quality improvement and safe clinical practice.

About You

You are a Registered Nurse who:

  • Is committed to recovery-oriented and person-centred care.
  • Demonstrates strong clinical assessment, risk management, and care coordination skills.
  • Understands the importance of physical health monitoring within mental health care.
  • Works effectively within multidisciplinary teams.
  • Provides culturally safe, trauma-informed, and least restrictive care.
  • Practices in accordance with National Mental Health Standards, Nursing Standards, and Local Health District policies and procedures
  • Is adaptable, resilient, and motivated to contribute to service innovation and improvement.

Working With Us

The Adult Mental Health Service is currently implementing a new Model of Care that moves from traditional team-based functions to clearly defined care pathways. This approach provides greater transparency for consumers, streamlines service delivery, enhances efficiency, and supports greater clinician autonomy.

  • Access to Transition to Practice Mental Health Nursing modules where appropriate
  • Comprehensive training in clinical systems and software
  • Ongoing professional development opportunities
  • A supportive and collaborative multidisciplinary team environment

Additional Information

All nursing positions are expected to work across the broader mental health service, including both inpatient and community-based settings, as required.
