Connected Care is part of System Performance Support Branch and provides expert advice on issues that may impact on health system performance,identifies opportunities for performance improvement, promotesthe implementation of state-wide patient flow initiatives thatimprove and enhance the delivery of high quality, efficient andeffective health services for the NSW community.
Key priorities for the team include system reform priories such as Urgent Care Services, statewide virtual care hubs and implementation of the NSW Health Virtual Care Strategy. The Connected Care team has a pivotal role in supporting and delivering initiatives/programs relating to collaborative care in the home, community and virtual settings.

About the role
With your skills and experience, you'll help improve health and wellbeing across NSW while growing your career within Australia's largest health organisation.
In this role, you will support the Connected Care team by delivering project planning, progress monitoring, reporting and coordination to ensure projects are completed on time, within scope and budget.
You will work with LHD, SHN and PHN stakeholders to support local and statewide initiatives and ensure alignment with statewide reforms. You will also prepare advice for Connected Care leadership and Ministry Executive on virtual care and other outofhospital care models.
As part of the Connected Care team, you'll contribute to key reforms that enable NSW Health to deliver more care outside traditional health settings through expanded virtual care.
This role is a Health Manager level 3, starting salary from $132,236 - $150,222 plus Super, with a 38-hour work week.

What you will be doing
- Provide expert policy advice and information to the Director and other senior executives on Connected Care priorities to support informed decision making and development of strategic reform related projects and strategies.
- Lead a range of management and support activities for Connected Care reform projects including preparation of reports, maintaining project documentation, and implementing and monitoring project plans to ensure project outcomes are achieved.
- Pro-actively plan, monitor, evaluate and report on all aspects of program governance and implementation, including risk and contingency management, governance, benefits realisation, and key milestones to ensure projects and outcomes are successfully delivered.
- Facilitate effective stakeholder engagement to empower co-designed solutions and initiatives to complex issues.
About you
We are seeking motivated and committed individuals who can support the work of NSW Health with:
- Can support large-scale health projects at a district or statewide level to embed reform initiatives including virtual care.
- Can support change management initiatives in clinical settings.
- Work well in a team, build strong relationships with stakeholders, and demonstrate excellent interpersonal skills.
- Have excellent written and verbal communication skills.
- Are agile and able to adapt to the changing needs of the health system.
