  • Location - Illawarra and some travel to surrounding areas further south on the NSW South Coast
  • Permanent part time role - 4 days per week, flexible with days and times
  • Pay rate – $54.47 per hour + superannuation
  • 7 additional leave days per year (pro-rata)

We have a great opportunity to join our Aged Care Assessment Services (‘ACAS’) team based in the ACT. ACAS positions offer flexibility and autonomy and are based across both metropolitan and regional locations, with most roles involving local travel within each region. We are currently looking for a Clinical Aged Care Assessor to join the team covering the Illawarra area with some travel further south.

What will your role look like?

You will be conducting assessments to identify and meet the complex needs of clients and refer to appropriate aged care services under the Aged Care ACT

Day to day this includes:

  • Collaborating with clients, families and multidisciplinary teams as required
  • Working collaboratively with colleagues to promote a supportive and positive team culture
  • Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and determining eligibility for home support services
  • Reviewing support plans to meet client’s changing support needs
  • Working effectively both autonomously and as a member of a broader team in delivering to key performance indicators within identified timeframes
  • Maintaining currency of knowledge relating to aged care services both funded and non-funded
  • Assisting with continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomes

To be successful in this role you’ll need:

  • Tertiary Qualification – in a health-related discipline directly related to Health, Aged Care or related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker)
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA)
  • An Australian Driver’s license and access to a registered, insured vehicle
  • Minimum of one year demonstrated experience in Australia or Overseas directly delivering services in Aged Care settings and/or to Aged persons including to people living with dementia
  • Willingness to apply for a Police Check
