The Connected Care Registered Nurse plays a critical role in delivering safe, high-quality virtual care through Remote Patient Monitoring (RPM), supporting patients to remain safely at home.This role is central to day-to-day clinical operations, including patient onboarding, monitoring, escalation of clinical risk, and coordination with medical and multidisciplinary teams. It directly supports timely intervention, continuity of care, and adherence to clinical protocols.By enabling early discharge, reducing avoidable hospital presentations, and supporting patient flow, this position contributes to improved system efficiency and patient experience. Hosted within the Triple I service, the role also supports the ongoing development of scalable, integrated models of care across Primary and Community Health.
Are You the Right Fit?
You are an experienced and adaptable Registered Nurse with a strong clinical foundation and a passion for delivering care in innovative, virtual models.
You bring:
- Proven clinical assessment and critical thinking skills, with the ability to identify and respond to deterioration and escalate appropriately
- Experience working collaboratively within multidisciplinary teams to coordinate safe, patient-centred care
- Strong communication skills, with the ability to engage patients, families and carers in a virtual environment
- Confidence using digital systems and adapting to new technologies, including remote patient monitoring

A high level of accountability, organisation and time management to balance competing clinical priorities
You demonstrate a commitment to safe, quality care, are comfortable working in a dynamic service environment, and are motivated to contribute to the ongoing development of new models of care that improve access, patient outcomes and system efficiency.

- Current Authority to Practice as a Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA)
- Describe your experience conducting clinical assessments and identifying early deterioration in a virtual care setting. How do you structure your clinical questioning remotely, use digital tools to inform decision-making, and escalate concerns appropriately?
- Describe how you have worked with a multidisciplinary team to support safe patient care in a virtual or community setting. Include how you contributed to avoiding hospital admission, supporting early discharge, or coordinating ongoing care
- Describe how you engage patients and carers to deliver person-centred care in a virtual environment (e.g. phone or video). How do you build rapport, ensure understanding of care plans, and support patients to recognise and respond to clinical risks?

Outline how you contribute to quality and safety in your clinical practice within a virtual or remote monitoring setting. Include your approach to documentation, use of protocols, risk identification, and maintaining professional standards.
