South Western Sydney Local Health District is seeking a dedicated, outgoing and purpose driven Social Worker to join the district-wide multidisciplinary Transitional Aged Care Program (TACP) team. This role is an amazing opportunity for a Social Worker to build on their experience by working with a diverse client base of older people living in the community. Through the TACP program you will be working with clients who have a wide range of challenges, and you will be able to work with clients with specific goals in mind. Whilst working with the clients you will have the opportunity to view their life as a whole and empowering them in their homes to achieve their goals. You will have the chance to work with families and the wider community to provide a holistic approach to the client care. You will also with work within a multidisciplinary team to deliver comprehesive care to our clients.

As the Social Worker you will have a keen interest in professional development, clinical supervision, teaching and quality improvement activities including service promotion and development activities. You will be working amongst a team of Social Workers in an Allied Health team. You will become a part of your client's life and see changes which will last a lifetime for these clients. You will be able to build positive working relationships with other community services and develop a rapport within the wider Local Health District services. Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

  • A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).
  • Demonstrated experience/sound knowledge of the current My Aged Care guidelines/systems.
  • Demonstrated experience/sound knowledge of the current processes required for RACF respite/permanent placement.
  • Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
