South Western Sydney Local Health District is seeking a motivated physiotherapist to join the community-based Bankstown Transitional Aged Care Program (TACP) as a Level 1/2 clinician in a full-time permanent position.

TACP is a specialised program focused on the reablement of older adults living in the community. It delivers short-term, goal-oriented care aimed at improving health outcomes and maximising quality of life for individuals transitioning back into the community following a hospital admission. In this role, you will work collaboratively with the multi-disciplinary team and relevant stakeholders to provide holistic, person-centred care to TACP clients and their families.

As part of this position, you will be responsible for conducting assessments in clients' homes, contributing to goal setting, and implementing client-centred treatment. You will support discharge planning and participate in delivering evidence-based care while developing your clinical skills within a community setting. You will also contribute to quality improvement and evidence-based practice initiatives within the team. With support from senior clinicians, there will be opportunities to further develop your clinical reasoning, participate in service development activities, and engage in ongoing professional development, supervision, and clinical education.

Selection Criteria
To help us assess your suitability for the role, please tell us as much as you can about yourself in response to the questions below.

  • Current Physiotherapy Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Describe your experience delivering physiotherapy services in rehabilitation, aged care, or community settings. In your response, outline how you manage and prioritise your caseload and deliver person-centred, evidence-based care.
  • Describe your experience working within a multidisciplinary team to deliver patient-centred care. In your response, outline how you collaborate with others to support goal setting and achieve positive outcomes for patients/clients.
  • Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
