Nepean Hospital is a teaching hospital that provides tertiary referral services in emergency care, coronary care, diagnostics, gynaecology, paediatric, neonatal intensive care/ICU, maternity, mental health, rehabilitation, and surgery. We are seeking a motivated Social Worker to join our Aged Services Emergency Team (ASET), a multidisciplinary team that provides assessment and interventions for identified patients aged 70 and over who present to the emergency department at Nepean Hospital.

Working within ASET, the successful candidate will provide senior clinical leadership, including identifying and enabling early interventions, identifying complex social issues that could impact patient length of stay in hospital and optimising patient care delivery.

This role is ideal for someone wishing to work part time between 14 and 17 hours a week.

What you will bring to the role

  1. Tertiary Qualification in Social Work and eligibility for membership with the Australian Association of Social Workers (AASW)
  2. Extensive postgraduate clinical experience in Aged Care and highly developed skills and knowledge of the key factors that impact on this cohort. This postgraduate experience would require highly developed assessment, planning and counselling skills.
  3. Highly developed skills in the areas of Crisis Counselling, Grief and Loss, End of Life concerns, Carer Stress, Conflict resolution and identifying and working with complex family dynamic's.
  4. Extensive working knowledge in the areas of Abuse of older people and NCAT Guardianship Division. Along with working knowledge of My Aged Care, Carers Gateway, Single Assessment Service and other aged care specific services and supports.
  5. Demonstrated commitment to professional development and demonstrated knowledge of and experience with quality improvement and evidence-based practice.
  6. Demonstrated effective written and verbal communication skills and the ability to work in multidisciplinary teams, including the formulation of complex care plans.
  7. Ability to provide advice to service managers on clinical development, practice and design
